A. with its registered office in Warszawa, ul.Chłodna 51, 00-867 Warszawa, KRS: 0000120139, NIP: 118 00 08-391, REGON: 010594457, before the expiry of the period provided for withdrawing from the agreement, being fully aware of the fact that the expression of this consent and the submission of such a request will result in me not having the right to withdraw from the agreement.

I agree to the storage and processing of my personal data for marketing purposes by Altkom Akademia S. A., ul.Chłodna 51, 00-867 Warszawa, in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (“GDPR”). Consent is voluntary. I am aware of the fact that I have the right to withdraw my consent to the processing of my data, rectify it, delete it or limit the processing.
